The tomb of destruction centers around a deadly curse, a debilitating disease that slowly kills everyone who has ever been raised from the dead, and prevents the resurrection of everyone who has died, and a soul merchant artifact that caused the disease. "The narrative of the campaign begins with the group being hired by Cyndra Sylvan, a wealthy former adventurer who suffers from a deadly curse. Her sources had tracked down a soul trader in the ancient jungles of Chult. At its core, Tomb of Annihilation follows the classic quest structure. The party has a goal, a deadline - Silvana is slowly dying, so every day that the party spends searching for her brings her closer to eternal death - and there is a dangerous stretch of wilderness in their path." This adventure module is designed to move player characters from level 1 to level 11.

The new module was announced in June 2017 during the live broadcast of the Annihilation event on Wizards of the Coasts Twitch. Tomb of Annihilation was released on September 8, 2017 as a 256-page hardcover book. It was also released as a digital product through the following wizards of the Coast licensees: D& D Beyond, Fantasy Grounds, and Roll20.
Tomb of Annihilation was inspired by the classic Tomb of Horrors adventure module, "a deadly dungeon created by D& D co-Creator Gary Gygax himself". Polygon reported that wizards of the coast has recruited more playtesters to try out the dungeons & Dragons Tomb of Annihilation module than any adventure it has released before. Why? Because they were afraid that no one would survive." The modular installation of Chult in the Forgotten Realms was last explored in the 2nd edition of Dungeons & Dragons.
In October 2017, Kotaku reported that Chris Perkins "said that no black writers or consultants worked on Tomb of Annihilation." About book design, Perkins said" we didnt want to create a city that felt backward.It is the story of how the sultans reclaimed their own city and land, redefined who they were, and moved from the idea of warring tribes to a more businesslike culture. The land they live in is a wild land. This is only part of what Chult is. This is a place of monsters. We wanted to show that the sultans not only survived it, but also rose above it - that they dispelled the warlike tribal nature that used to define them and is now really flourishing."
On June 17, 2020, in the Wake of George Floyds protests, Wizards of the Coast announced: "when each D& D book is reissued,we will have the opportunity to correct errors that we or the broader D& D community have discovered in this book. Every year, we use these opportunities to correct many things, including errors in judgment. For example, in the recent reissues of the tomb of destruction and the curse of strahd, we changed the text that was racially insensitive. These reprints have already been printed and will be available in the coming months. We will continue this process, reviewing each book as it comes out for reissue and correcting such errors where they are present."

Springer wrote: "when they stack up against the thunder storm king, Tomb of Annihilation feels blessedly streamlined and adventure-oriented. There is no policy to navigate, no siege to withstand the weather - it is just a deep dive into the unforgiving desert to destroy an artifact that cannot be destroyed."
Cameron Kunzelman, for example, emphasized that this adventure is unique compared to previous modules and that the chult jungle is designed to try to kill you. Kunzelman wrote: "however, it is important to note that the jungles and dangers also slightly remind them of the darkest Africa, and the people of Chaltan are dark-skinned. The narrative rhythms in the tomb draw heavily from familiar sources such as Edgar rice Burroughs or Jules Verne, and these works have a long racial history that is not often encountered. While the literary and visual content of the tomb borrows from it an interesting design and visual effects, I think the magicians could have done more to interrogate the texts they borrowed and how they have a complex past with General ideas about Africa writ large."
Cecilia Anastasio of Kotaku stressed that the adventure uses "outdated stereotypes of African cultures" and themes of colonialism that go back to the creation of the Chult setting. Anastasio wrote: "heres the catch: while many of the players I spoke to enjoyed the way Chults story and political structures were expanded in Tomb of Annihilation and enjoyed the adventure plot as a whole, they were still not impressed with its execution. His installation is a fusion of African cultures, a trope often found in 20th-century media that flattens the dimension of human experience on a continent that contains hundreds of ethnic groups."
